Certification Visit

During your certification visit, your medical conditions will be reviewed to verify eligibility.  This verification must be obtained prior to applying for a Kentucky medical cannabis certification card.  It is important that you provide any supporting medical documentation substantiating your qualifying medical condition(s).

Kentucky state law requires first visits to establish certification be conducted in person.  We will see you in our Elizabethtown office for this visit.  Medical certification is valid for 12 months. Subsequent visits and support options include face to face, telehealth or asynchronous virtual health.
